Creekside "All Sports" Summer Camp Participant Waivers

I understand that I/we are being allowed access into the facility and do so by free will. I understand that Creekside “All Sports” summer camp is a family friendly facility and that I/we are expected to abide by all policies and procedures when engaging in any activities within these doors. I understand Creekside “All Sports” summer camp may use, publish, and display my/our likeness(es) in photograph(s) or video(s), that Creekside “All Sports” summer camp has or will make while I/we are at Creekside “All Sports” summer camp.

I understand there is an inherent risk of injury from activities at Creekside “All Sports” summer camp. While particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown and assume full responsibility for my participation. Additionally, if I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.

I understand that using a facility in which other people have been present exposes me to the risk of contracting communicable diseases including, but not limited to, Covid-19. I understand Creekside “All Sports” summer camp will engage in reasonable cleaning practices to reduce the risk of exposure to communicable diseases, but Creekside “All Sports” summer camp will not be held responsible if I should contract a communicable disease that I believe I was exposed to at Creekside “All Sports” summer camp. I will follow applicable rules regarding the cleaning of equipment and facilities after my use. If I believe I obtained a communicable disease at Creekside “All Sports” summer camp, I will inform Creekside “All Sports” staff as soon as reasonably practicable. Additionally, I will not use Creekside “All Sports” summer camp facilities while ill or having a communicable disease.

I certify that the below named persons are physically fit, have sufficiently prepared or trained for participation in the activities or events they will be participating in, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my/our participation in Creekside “All Sports” summer camp activities or events.

I grant to Creekside “All Sports” summer camp permission to email me about activities and events happening at Creekside “All Sports” summer camp as well as special offers from the Creekside Christian Church. Creekside “All Sports” summer camp promises not to share your email address with any third party including their partners.

Creekside “All Sports” Staff will administer medications to participants who have asthma or who experience allergic reactions. Creekside “All Sports” Staff will NOT administer blood-glucose tests or insulin shots. However, Creekside “All Sports” Staff will store blood-glucose tests and insulin for the participant to administer themselves. Any substitute foods for raising blood sugar, such as honey, orange juice, or other food substance, will be maintained at the parents’ request if we are reasonably able to do so.

Parents of children with any potentially life-threatening illness or condition must carry a pager/mobile phone with them whenever the child is at Creekside “All Sports” summer camp.

Epi-Pens and other medications will be checked in with the supervisor upon arrival. The child’s name and allergies/medical concern will be written on a label and placed in the medicine container (a plastic box that closes and is kept in the “common area”). The medication will then also be placed in the container where supervisors can easily get to it. The child who has the allergy/medical need will get a red wristband and on his/her wristband will be written his/her name and the medical concern. At the end of each day, parents are responsible for picking up their child’s medication and returning it the next time. If the medication is not picked up at the end of the day Creekside “All Sports” summer camp is NOT responsible for medication storage.

No child will be allowed to hold onto his/her Epi-Pen or medication in personal bags or lunch boxes. All Epi-Pens and medications must be given to the morning supervisor to be placed in the secure medical box.

If an Epi-Pen is administered or other urgent medical need arises Creekside “All Sports” Staff will call 911 and then the parent and/or emergency contact. The parent will be responsible for any costs associated with the call and treatment of the child. Parents will be notified in case a child needs an inhaler or requires assistance pertaining to blood sugar.
